From March 2001 (Vol 11, No 2) Journal of Military Ordnance,
in which all DU incidents (M-1 penetrations, fires, friendly fire tank
round hits on other vehicles etc) are listed for the Gulf War.
Task force 1-37 Armor, Evening Feb 26, 1991

Bumper # C-12: Blue-on-blue hits on M-1 tank, 120mm DU round hit
in the rear hull, disabling the engine, followed by AT missile hit
on turret bustle area. No penetrations of fighting compartment.
Vehicle was temporarily abandoned but did not burn or explode.
Battle of Norfolk, Early Morning Feb 27, 1991

B Company 3-66 armor
Bumper # B-66: (company cdr tank) hit by 3 US DU rounds, one penetrated
the turret ring and killed the gunner and injured the commander,
the second penetrated from un-described angle injuring loader and driver
who were attempting to rescue the gunner. Vehicle burned and exploded.
None of the hits were on the special armor areas.
B-22: was hit shortly after B-66 (was turning to help B-66 at the time)
by a US DU round on its mine plow and front slope, the driver was
slightly injured by flash burns from the impact and an electrical
short circuit from the damaged plow started a small fire inside
the tank, though the tank didn't generally catch fire and was
repaired in the field.
A Company 3-66 armor
A-14: hit by one 120mm round from unidentified direction,
causing injuries to the driver, loader, and gunner and
a fuel leak which eventually caused the vehicle to catch
fire and burn.
A-31: Struck glancing blow by DU round, holing fuel cell.
Not disabled, but was turned in for repairs.
A-33: Struck from behind by AT missile and disabled, while crew
were evacuating two DU rounds hit tank, entering left side
and exiting on the right side, injuring the commander, driver,
and gunner who had not yet exited the vehicle.

A company 3-69 bumper A-66 caught fire due to transmission
problems and destroyed due to ammo explosions and fire.
Feb 26, 1991 fire due to non-DU munition impact

B company 1-37 Armor
B-23 struck from behind by large shaped charge weapon,
penetrating the rear hull doors. Second round struck
while crew was evacuating. Penetrations caused a catastrophic
hull fire and ammunition explosion series which destroyed the tank.
Evidence that the shaped charge weapons were Hellfires,
but not proven.
April 4, 1991 Accidental Tank Fire

D company, 2-34 Armor
D-66 caught fire during tactical road march, forcing crew
to abandon vehicle despite using all onboard and available handheld
fire extinguishers. Vehicle burned for 2 days and then ammo cooked
off in the hull ammo compartment. Apparently the bustle rack ammo
was undamaged and did not explode. Damage was mostly confined to the
hull ammo rack and the engine compartment burned out.
April 13, 1991 Accidental Tank Fire

A company, 2-81
A-31 being towed by A-32 caught fire due to turbine engine exhaust
overheating bustle ammo rack and causing several rounds of ammo
to explode. A-31 burned out.
